Services
The Transition Care Program (TCP) provides low level therapy and care for up to 12 weeks, for people over the age of 65. People can be admitted to the program in either a residential bed or receive therapy in their own home. Admission to the program must follow a hospital stay. The program allows people to take more time in a non-hospital environment to complete their rehabilitation, or to achieve the best possible recovery. It can also support families to finalise longer term care arrangements, including returning home with supports, and residential placement.
The program provides therapy which may include physiotherapy, occupational therapy, speech pathology, nursing support, social work, podiatry and/or dietetic advice.
In addition, services provided to community-based clients may include personal care, home care, in-house respite and meal delivery.
The ERH program is part of a formal partnership with Bendigo Health Care Group. ERH offers two streams of TCP, including a bed based facility in Loddon House Glanville, and a community based program where therapists travel to clients in their own homes. ERH currently has six residential places and four community based places.
Eligibility For TCP
- Currently an inpatient of hospital
- At least 65 years of age
- Medically stable and ready for discharge from hospital
- Eligible to enter low or high care permanent residential care
- Assessed by the Aged Care Assessment Service (ACAS) as suitable for TCP
- Community based clients reside within a 20 km radius of ERH
- Would benifit from the program
Fees
Client contributions for TCP are currently 85% of single aged pension per day for residential clients, and 17.5% of single aged pension per day for home based clients.
