Services
Community Palliative Care provides specialist palliative care nursing support and education to clients, family and carers in their homes. Echuca Regional Health provides a multidisciplinary approach to holistic care for people with life-limiting conditions. Our multidisciplinary team comprises Social Work, Occupational Therapy, Physiotherapy, Nursing and others as needed.
The Palliative Care Nurse Specialist will: -
- Assess your condition and discuss your needs to develop a plan of care with you and your family.
- Advise on managing symptoms of the illness.
- Assist with the hire of aids and equipment that may be improve the care for a client at home.
Care is given in conjunction with General Practitioners and Specialists, Community Nursing and other support services.
The Palliative Care service provides support and advice to other care agencies within the Shire of Campaspe
Referral
The Community Palliative Care Service accepts referrals from General Practitioners, Primary Care Centers, Echuca Regional Health any other hospital. Referrals from community members or self are also accepted
Referrals using SCTT template are accepted via fax, post or e-referral using the
Connecting Care website.
